it shouldn't be shocking that the profit motive on commodities with inelastic demand (as in, consumers have to have them no matter what and will have to pay any price they can for it) like oil/healthcare end up being low quality. Since these are capital-intensive areas, under capitalism, larger companies that can afford to invest in more capital succeed in the short run by being able to edge out competitors in price, which results in the existing market concentrating into monopolies or ologopolies. Once market share is dominated by a few, then quality drops on basic care/ what the majority of people will be able to get because it's not like people can shop around. Those companies are accountable almost exclusively to shareholders (regulatory capture helps with this) so the more corners you cut, the more profit you get. Charge 15 dollars for an aspirin, no one can question it. This is why governments take over with a natural monopoly on things that are inelastic and require expensive infrastructure that doesn't benefit from redundancy/competition.
