No, I'm pretty sure they're all in the movies. I can only verify the one from my post. I remember watching TMWTGG on TV for the first time when I was 13 or 14 and even then I just remember thinking, "What the gently caress?" when they played a drat slide whistle and basically ruined this really great car stunt.

The problem is, it's sort of a nudge and wink to the audience as a reminder that previous Bonds had some sort of rejoinder or quip for every situation. The setup and delivery in QoS were both great, but you couldn't really pull that off a second time (or more) and expect the tonal shift that we've had with the Craig Bond movies to really stick.
