I thought it gave you a chance. I know there were runs before I got the D6 that I would always gun for that book and spam it for the hearts. I would occasionally not fight the horseman, and thought the wiki always phrased it to be "increases your chance to fight" one instead of "will fight" one.

So I just checked and oh hey. It doesn't work on XL floors. What do you know about that.  Other than that, it should definitely force horseman to spawn. Also remember that Headless Horseman counts as a horseman, too. (10% chance of replacing another other horseman) If it isn't spawning horseman, well, there's a bug buried somewhere and I'm not too surprised about that at all! Well I don't know if that's what it was, or if I did actually get a level spawned without one once now. I'll have to try to remember in the future if I do that again.

Son of a bitch, I just got screwed out of a great chance at no hitting Depths and Womb cause of Blue Mom and the Bloat. I even got cat mode for the first time. God I hate the Bloat, it's ruined so many great runs by itself.

Well, pretty sure the Book of Revelation only gives you a very high chance of getting a horseman, since I just used it on a normal floor and got the Hollow instead. I did end up making a Super Meat Boy that game though. FINALLY! ...except the game didn't give me the achievement. loving hell.

It'll give it to you next time you launch the game, hopefully. Isaac is sometimes slow to respond when it comes to Steam achievements.

More than one horseman can't spawn per set of floors, so if you've already encountered one the Book of Revelations won't work.
