I think it's indisputable that EoE was originally supposed to be episodes 25 and 26, but I doubt some parts of it were planned all along. The hospital scene in particular feels to me like something Anno put in just because he was disgusted with everybody and wanted to make Shinji less likable. And I doubt they'd let him show cum on TV like that
