after some consideration, and not wanting to repeat some of these other fine and valid answers, I'm going with An American Werewolf In London

The way Gere's wife played the hospital room scene with the blood vessels broken in her eyes after car crash: that's how you do scared.

The Lovely Bones was also creepy. Whoever played that pedo serial killer rapist did a pretty good job at portraying a pedo serial killer rapist. Don't think its a horror film though.

Horror or not, I've never been so scared of a movie since this one. I'm not sure what it is about that movie, but it still freaks me out when I think about it. It has just the right combination of weird, unexplained poo poo, and momentary flashes of "wtf was that?" that just kept me feeling uneasy. Does anyone have suggestions of movies like this?

people have some weird criteria for "not really a horror movie" i mean the mothman prophecies is a movie that primarily aims to instill a sense of dread, about a supernatural creature that manifests right before disasters happen sure it's trying to warn people, not cause the disasters but that's not really spelled out till the very end and is it really enough to disqualify something for in the first place?

Not the Butcher Boys. However, if you judge the quality of your horror movies by whether or not there is a lobotomized cannibal(?) using a rocket launcher, the Butcher Boys is the best.

When I think of horror films and have conversations about horror films with others, more often than not the kinds of movies that use extreme gore and suddenly loud and startling events are what they are talking about. Stuff like Friday the 13th, Saw, a lot of zombie stuff. It's a very visceral reaction as opposed to the psychological dread of movies like The Mothman Prophecies. I guess we should add another sub-genre to the horror list. Psychological suspense? Pandorum was pretty good too in my book.

the best psychological horror movie is either jacob's ladder or cat people (the original)

words like "suspense" and "thriller" get a little murky in their definitions but yeah there doesnt have to be any supernatural or fantastical stuff in a movie to make it unsettling
