You'd have to do things like stuff garbage back into garbadors and surgically remove magnemites who have accidentally fused with electric pokemon, and even perform procedures to remove pikachus from people's assholes. yeah that game would be pretty sweet i'd play it constantly.

I had an idea like this ages ago where youre a pokemon brain doctor slowpokes dim intellect and snorlaxs sleepiness are all caused by brain tumors and you need to perform delicate brain surgery to cure them also you need to perform a lobotomy on psyduck for some reason
