Right? Like, I get that the joke was probably supposed to be that she had the wrong idea about what an enema was, but I don't know how that happens and its a rather gross joke compared to most of the humor in this show.

I think the idea is more that Aoba has a weird fixation with appearing adult, so much so that she's totally psyched to go through uncomfortable medical exams older people have to deal with.

This was exactly what I got out of it. Considering the other times she was trying so hard to be an adult(ordering alcohol, buying tickets as an Adult vs as a Student, etc) it seemed pretty obvious that was the episode's "I'm an adult now!" joke.

I suspected that was a translation issue. Still, the whole checkup scene felt really off to me - a doctor's visit is a weird and uncomfortable framing device for fanservice, IMO. Could easily have accomplished everything this episode did with a weekend hot springs visit or something.

Yeah it's basically just that it sounds strange/funny to Japanese people not from Ryukyu/Okinawa. It's a stereotype a lot of people have about Okinawa, that they have really weird names, so she's probably more than a little self-conscious about it.

Also yeah, a lot of people have trouble reading Okinawan names because they tend to use different readings for the kanji than a more "typical" Japanese name. For example the "gon" in Ahagon is almost always read as "ne" when used in surnames throughout most of Japan.

As an aside the name basically means "root(gon) of the waves(ha)" (the first 'a' sound doesn't really mean anything, though it may have at some point).

She probably grew up in Okinawa, where she and her name are from, and therefore wasn't teased for it. Then she comes to... Whichever part of Japan they're in, and people keep commenting on her "weird name" and pronouncing it wrong and stuff.
