it's both. the widely covered crashes from recent weeks stemmed from issues you mentioned, but manoeuvres like maciejuk's in rvv 23 still occur multiple times a race

In this case there’s a kerb or something, unmarked and with no barrier, that runs right through the racing line you’d plan to take, before you’re able to see the kerb. He was committed and going too fast to stay in what was suddenly a much narrower road than expected. In the big Tour of the Basque crash, the road surface right on the corner was ultra crappy with roots under it right on the corner, no warning. One person lost it, then some of the others that crashed were just due to reacting to that. Consequences were so bad just due to the ditch in exactly the wrong place. Any other cases you’re thinking of?
