I think it's probably even more freeform than that, they probably just come up with a concept and run with it from there. In their Onion A.V. Club interview, they said that something like 5 different Dr. Steve bits came out of two hours of goofing about.

I dreamed that I bought The Innernette at Dollar Tree. Apart for a few problems with installing it on Vista (had to disable UAC), it was surprisingly good, aside from chat. It took 200MB of disk space. Then a plane crashed in front of my house, and I lost interest.
