Well Annihilus looks a lot smaller than he normally does so he might not be fully grown yet. I imagine it's after Vulcan attacks the planet that was keeping the larva in a stasis field to keep it from growing.

When was Quasar killed? In the original Annihilation: Nova miniseries, which took place before Annhilation itself. He came back later in the Nova book.

Yeah. The miniseries takes place in England, where we see what Batman's adventures would be like if he was from there. A lot of it is Cornell creating as many British heroes and villains as possible, like Milk Man, Death Dinosaur, Coal Miner, Double Entendre, Birthday Girl and the above-pictured Jarvis Poker the British Joker. Despite being considered a villain, he admits that he doesn't have it in him to commit crimes. He just really loves the Joker's style.
