Last year someone in my brigade got shot with an M4 and I've been present on a SAW range where a gun malfunctioned and got walked off the range with a live round stuck in the chamber. I'd be curious to see statistics Army-wide for NDs resulting in injury or death.

I think my favorite on KAF was either the AF member who ND'd their M-16 into the engine block of a pickup because they thought it was a good idea to be cleaning their weapon while driving down perimeter road or the Army Lt Col who ND'd into a Lt. The AF member who ND'd during a function check because they dropped the mag from their rifle after pulling the bolt back to clear it () was pretty lol too.

I couldn't even tell you the amount of times I had to guard something sensitive like ammo with m16s and hadn't been issued even a dummy magazine for looks. Never had a ND!

I don't know about issuing weapons but I am fully for Army personnel on domestic bases being required to wear full SAPI plate, helmet, eyepro at all times (plus reflective belts)
