I don't think you get penalized if you fail a goalie interference challenge in IIHF, you only lose the timeout. That's how it was in NHL too before this season. I can't imagine what this last one was if not a coach's challenge from the Czechs.

If the Coach’s Challenge does not result in the original call on the ice being overturned, the team exercising such a Challenge will forfeit its Time-out. So no penalty. You only get 2 minutes if you challenge for an offside play and lose.
