To contrast them with Josuke it's possible that the Head Doctor and Tooru are the result of the rokakaka splitting one person into two people. Considering that the Head Doctor made a point of asking the reporter if he was pursuing him shortly before the reporter died it's pretty likely that the Head Doctor's Stand is the pursuer-targeting calamity attack and not the robot with the bone blades that ambushes people. If that was the case then I don't think he would have had to use the rokakaka exposure to decapitate the reporter.

I feel like it's pretty clear right now how the calamity stand works. We've seen that, apparently, it has a need to travel; it cannot do anything if people can't see it, so it had to do all those whacky shenanigans with mirrors and glass doors to travel towards the bed, and gently caress up Jobin. If you can't see the stand, but you're still under its effect, the only way you could be attacked is by running into things by yourself.

What this doesn't explain is how the stand appears before multiple people. Maybe it can just affect multiple targets at once, gently caress you, that's it, everybody gets their own unique instance of the Head Doctor throwing extremely specific bad luck at them, engineering situations just for that. But as others mentioned, there may be more to that.

One thing's clear - not that anyone doubted that -, Tooru is evil because he seems to have no intention whatsoever of helping
