It looks like this is a trade school situation and the instructor was leaning too hard against the mock-up wall and pushed it over. No mortar because it's a practice wall and they want to reuse the bricks for the next student. That doesn't change the fact that this wall was 3-4 courses too tall to not have mortar in it. Even super-thinned out mortar that never actually dries (training mortar) would have helped.

Anglo, I dunno about now but they used to be a bunch of loving cowboys. They nearly killed those guys in 2009, killed a guy with an exploding tyre in 2010 and then killed three people in a 12-month period at that mine and one nearby in 2014-2015.

We had a guy do this, in a trailer, on the left side (less stable since roads aren't flat), without any shrink wrap holding the barrel to the pallet. He'd been with the company ten or so years at that point. He's since been promoted about eight times.

Yeah that's the video and that voice over is done by Phil Lentz over in Perth. http://lentzmedia.com.au/ He is a great bloke to work with. Imagine 5 years of editing to his voice. Then you get him on the phone and he's Mr Campy Gay GuyTM with his regular voice. One tactic of his was to get drunk and smoke ciggies the night before to gruff up his voice a bit. Doing the VO hungover. I MIGHT be confusing him with Noel Hurley who did some VO stuff from time to time.

Yeah there's a lot of questionable decisions made in this process but in my opinion, this is the big one that leads directly to the injury. I also agree with the narration that "when you encounter a mechanical interlock that is there to stop you doing what you are currently trying to do, maybe you should stop and consider if you're doing something dumb"

I dunno dude they're drilling and tapping into the switch and bypassing a mechanical interlock. At that point as a designer I'm calling it officially "on them". Yeah, the spinny thing is the strands that it's winding together into a cable. It's a cool design that takes up half as much floor space as putting the spooling stuff somewhere on the outside. But it's also kind of insane.
