People got in a bad habit of leaving the tugs on and in gear and just using the parking brake when making a quick stop. He went to open the cargo door and a bag fell, hit the parking brake lever and the tug started creeping towards the plane as he had his back turned. He then saw it almost at the plane and tried to push against it to stop but ended up crushed between the two and died. Someone could wind up with a lot of psychological baggage from witnessing that.

You'd need to dam and divert a river, conduct a deep excavation through solid rock, drain the pipeline so you can conduct repairs, and then close everything back up. It literally might be simpler to get a tunnel boring machine and dig a parallel pipeline. (Presumably that's how it was laid that deep in the first place).
