re ADHD chat: if you've been on the same medication for a very very long time ask your doctor if there are any newer better treatments available.

i was diagnosed with ADHD when i was 10 and saw a specialist paediatrician until i turned 18 and then i started seeing a psych. that dude prescribed me compounded slow-release dexamphetamine from when i started seeing him up until ~2020 when he abruptly retired. in the whole 14 year period that i saw that psych he never once suggested alternative medications and just kept me on the compounded slow-release dex which seemed to work okayish.

in my first appointment with my new psych he suggested ditching the compounded slow-release dexamphetamine and instead taking lisdexamphetamine (vyvanse) with regular dexamphetamine. i'd never heard of vyvanse before but it sounded pretty nifty so i tried it out and it was a marked improvement over the compounded poo poo. it was also much cheaper (didn't have to pay the ~$60 compounding fee), easier to get (compounded stuff i could only get at PCCA compounding chemists) and more convenient (just one 70mg vyvanse per-day instead of 4x10mg compounded capsules lol).

so yeah, it's a shame that it took having to change doctors to be made aware of the latest advancements in drugs. ofc i guess i could have been more assertive with my original doc but he was a bit poo poo in general tbh.

my two younger siblings also got diagnosed with ADHD in childhood but they both stopped taking meds after turning 18 and seem to be doing fine. i on the other hand cannot function without them lol. been on the drugs for 23 years now im certain my brain is porridge or something

vyvanse is super expensive here unless you get one of those coupons (it knocked like $150 off to make it $10 for me, at least)

i dropped it because i didn't see any benefit over regular extended release dextroamphetamine, which is free, so i stopped taking it. once they stopped giving out the coupons my psych was like "yeah we don't prescribe that anymore", to which i lol'd heartily

that usually means they got a kickback per rx. i don't know how it works by you but i'm glad you found something that works, adhd can be a bitch to manage

yeah, I wasn't too keen on the meds I had in my teens. they'd make me hyperfocus, but not necessarily on something productive. and the appetite suppression wasn't healthy.

edit: there's no discount on the regular dex that i take alongside it however overall it's still cheaper per-repeat than the compounded slow-release dex which was like $40 for 100x5mg dex plus $60 for compounding.

i was diagnosed with add as a kid and did the ritalin/adderall thing through middle school or so. i've always massively self medicated with fuckloads of coffee and tried going back on adderall in my mid 20s. i had to quit again after a just a couple months because while it was effective, it also made me aggro as gently caress, and left me constantly wanting more drugs. in my late 30s now and while i'd love to give it another shot, my cardio system isn't in the right place for it.

Coming out of my cage and I've been doing just bad
I got the aggro thing from Vyvanse but not Adderall for whatever reason. Think I'm mostly in a good spot med wise at this point, except I tend to run out of steam about an hour or two before the work day is done. Not really enough time to warrant upping my dosage, and XR just works significantly less effectively for me while disrupting my sleep way more, so I'm kinda just rolling with what I've got for right now.

they actually were going to switch me from adderall to vyvanse when i was complaining about the aggro thing, but I never filled the vyvanse prescription and just went off meds.

for the last decade the Liberal/National coalition (basically aussie tories) has been in power and have been gutting the PBS and medicare. im certain that if they got another term they would have privatised medicare. thankfully they lost the federal election in may and we now have a majority Labor (union-backed party, like UK Labour but not total dogshit) government. here's hoping for a thousand year Labor government and that they resurrect Gough Whitlam to lead

i was also diagnosed as a kid but had no idea, my parents literally hid the concerta rx i was given and i only found it when i cracked their safe (yeah i was a little bastard). it's a big ole hit of dopamine which would explain why you'd constantly want more drugs, and amphetamines are themselves crazy addictive. i'd also self-medicate with tons of caffeine and pseudoephedrine

sometimes i worry about the cardio effects too, but my resting heart rate even on amphetamine is in the 60s so i hope i'm fine

the running out of steam issue is why my doc prescribed me vyvanse and regular dex. in the morning before work i take 1x70mg vyvanse and 1x5mg dex so that it kicks-in quicker and at 2-3PM in the arvo i take 1x5mg dex which keeps me going till at least 5-6PM. works well and lets me get chores and poo poo done when i get home from work before semi-crashing.

ive been taking some form of amphetamines almost daily for at least 23 years now. my resting heart rate is usually a bit elevated when i take them however i had an EKG recently as part of a general check-up and they didn't find any issues. so yeah you're prolly fine.

semi-related but im now like 7 years smoke-free after a ~10 year habit which began in high-school lol. im surprised i managed to pull it off, was hard to imagine when i was smoking. i used to do off-shore work on the reg and would wear patches when i was out on the rigs. after one swing i just decided "gently caress it ima keep wearing the patches and go from there" and it worked. only good thing ive ever done lol

yeah, my issue is that i'm already sitting on a long-qt, high blood pressure, and nearly tachy resting heart rate. until i get all that under control stims just aren't an option. i'm not even allowed pseudoephedrine to clear my snotty sinuses out.

as it turns out, years of abusing your body causes it to start failing pretty early

Dialing in my meds was tough (and is still an ongoing process somewhat at 5y). With the psychiatrists available to my insurance, you have to bully them into listening to you.

My kid (actually the reason I got diagnosed as an adult) got recommended a psychiatrist who was not on any insurance, and she was expensive but god drat she was good at figuring out what needed to change in prescriptions, especially for a young kid. But alas, she retired and now I'm not unhappy with my kid's current provider, it just feels more like they're reading from a checklist at every appointment.

EDIT: My borderline high blood pressure was not affected by stims. My psych was super concerned about it, but my primary care provider was not. Like we were monitoring before and after starting Vyvanse, and the numbers stayed bad but didn't get worse.

after i found out my parents had been lying to me and became an adult i just went to a psych on my own and was prescribed adderall, that was probably 18 years ago or so. still, after i took some after a killer hangover i sort of had a panic attack and my heart rate was in the 130-170 range so i went to mass general hospital to get checked out. they did an ekg and i was totally fine, but that's how i'm now on benzos for anxiety also lmao

and yep i quit smoking in 2019, right before covid hit ironically lol. replaced that with vapes which imo probably isnt too much better but :shrug: that withdrawal makes me aggro as all hell

i didn't think i could take stims for a long time because of my history of pretty severe panic disorder, but as long as i have something to kinda smooth out a stim, it's fine. I took propranolol concurrently for a couple years, and now i take guinfacine concurrently with adderall. it lowers my heart rate similar to propranolol, but with the added benefit of also being an ADHD med

my meds struggle atm is that adderall XR works fine for me, but IR makes me feel kinda nuts, like it just dumps into my system and makes me feel strung out, and my pdoc had me start taking it as a sorta booster alongside the XR. I just do not like it. >:(

I self medicate with 600mg of caffeine and a vape and it does ok. my resting heart rate seems to hover in the 55-65 zone, and my Apple Watch tells me every morning that my heart rate hit 35 or 40 while I was sleeping, so either I have athletic bradycardia or I somehow have sleep apnea while being pretty slim and fit

doctor wasn’t too worried about the slow heart rate while sleeping so ¯\_(ツ)_/¯

i'm on propranolol right now for interview performance anxiety!

vyvanse was great but i literally could not sleep and when i would i would "think" in my sleep anyways so it wasn't real sleep. adderall IR is ok and lets me get to sleep but i wish i could get the vyvanse effect in a shorter time period.

the day i did my sleep study to set up my cpap pressure i felt like i had super powers after taking vyvanse.
