She was never there for her young child while she was dying alone in a hospital. That's pretty loving evil and she'd need a drat good excuse for it -- like, a "being held hostage by the murderer" kind of excuse. Thinking about it, the implications of her mother having been involved in the entertainment industry opens up the possibility of new twists and connections, possibly disturbing ones. If I recall correctly, Sarina and Ai were about the same age and Sarina knew about her before Ai really blew up -- and 12 seems really loving young to go big, even for Japan. he actually says  i think ai says she's a second year middle schooler when she gets scouted (13-14 in japan), so likely ai and sarina would have been a grade apart in schooling but are still only months and not a full year apart, and ai probably debuted at 13. as an aside on the topic of pre-reincarnation ages, aqua describes his former life as just '30s' in the english tl, but in the jp the term he uses is 'arasa,' or 'around thirty.' the exact band it refers to is generally 28-35. given gorou's residency was 4 years ago, he was probably 30 or 31ish.

I stumbled across the love and lies theory and it certainly seems like it'd make for a fun and wild development. It probably isn't true, but it'd be hard to do a proper gamechanger ending that recontextualizes everything up to that point better than the broad strokes that the theory puts out there. I do think us dumb readers will be explicitly told what the different colors of eye-stars symbolize at some point, and that it won't be quite as simple as black star eyes = wanting revenge for Ai's death.

My recollection of Japanese healthcare from a course on Post-WWII through Present Japanese Society I took is that while the most basic level of care is cheap, anything above that will cost extra, often paid under the table. Want any doctor other than the most sleep-deprived resident? That'll cost, especially if you want the best doctors at the hospital. Want a private room rather than a bed in an open ward? That'll cost. Better sheets? etc. Also access to specialists is very difficult to come by (having influence and/or connections to the owners of the hospital helps a lot), and for rarer, more complex issues you may well have to look abroad.
