You could look into a drafting style setup. The tables can be adjusted pretty high easily to accommodate standing, and you can get a bar stool or drafting chair to fit the desk.

Then I just checked around for a table at that height and a chair to fit.

Edit2: There's a $400 stool that is also supposedly designed to strengthen your core by making you fight to stay upright. http://www.wired.com/gadgetlab/2011...-has-your-back/ It's made by the Dutch, whose milkmaids know a thing or two about stools.
