http://bindingofisaac.com/post/155355804054/afterbirth-is-out but its not actually live yet. but there is a cool update here where this is the last expansion mcmillen is gonna do but from here on out he will take some of the best mods (new items, enemies, bosses and challenges) he finds and patch them into the game every month with a mini update, which is a cool idea.

Think I figured it out, since it's updating now. Right click game -> properties -> DLC it will have Afterbirth+ checked off but not installed. Uncheck then check again to dl it

I thought I was using it wrong for a bit until I realized my controller had to be remapped with the update and I didn't actually have a Use Item button lol.

Initial thoughts on Afterbirth+: it's annoying as gently caress. There are way too many enemy spawning vortexes right out of the gate when you have 2atk. It reminds me a lot of Wrath of the Lamb dlc in the original flash game, where they introduced the cellar and spider sack enemies. When it first came out there were like 500 spiders on the first 2 floors that made it near unplayable. I hope they tweak the spawnrates on them asap. The new character seems alright but the cooldown on his spacebar item still makes me want to just take the thing I want instead of sucking it up. Haven't tried Greedier mode yet. Only 5 new challenges?  Coupled with the insane installation problems (friend of mine still cant play it and has tried every fix) makes me feel lukewarm towards this update.

Apparently theres an AB+ achievement that you get if you reset too many times consecutively that turns soul hearts into half soul hearts. Im not buying this
