From what I understand, it's based on fixed time intervals, where a certain amount of time in-game is supposed to pass before they change any regular 30-second timeouts into TV timeouts. But the network is always free to run commercials after scores, and they can try to cram commercials into any officials' timeouts if they want to. The networks only guarantee a certain number of time slots to advertisers, so if they get a bunch of TV timeouts in early, they might run fewer later. But I don't think they specifically have to.
