How is everyone just living their lives like they're not in the middle of a war? Because they're not in the middle of a war, the handmaidens are a right reserved for the upper class of the society. The same upper class that want nothing to do with the actual fighting so are well and truly insulated from it. The war is an abstract for everyone in the show, as evidenced by the oranges scene in the first episode.
