It's also not really clear what Dr Sivana wants, once he gets magic powers and revenge on his abusive family. He's already a rich doctor. I guess he was gonna rule the world?

Sivana I think comes off as under-explained when it comes to his plans and motivations when we're used to comic book movie supervillains having very personal and spelled out motives. But that he wants power for power's sake is probably all you really need- we don't know what he might do left unchecked, but it's probably not going to be good, and he might be bad enough that the Sins will gladly do his bidding because it's worse than what they'd get up to on their own. The abusive family and rejection by Shazam is apparently already a significantly more fleshed out backstory than Sivana usually has- as has been said, he's the guy Lex Luthor stole most of his schtick from, THE comic book mad scientist fuelled by spite and envy, and they've never really tried to make him sympathetic when he's far more fun as a complete lunatic who just never gives up his plots and schemes.

Yeah, as cliche as that might have been, the movie could have used a scene or two showing the Sins' effect on people other than Sivana. On the other hand, a bitter vengeful prick being their avatar kinda says everything you need to know about them. There is a deleted scene where Sivana crashes his family's big Thanksgiving party and murders everyone there too. They probably should have kept that to drive the point home just how much Sivana loathes family as a concept, and the threat of his and the Sins' rule would be the rather abstract elimination of familiar warmth and whatnot rather than a concrete worldly goal.

Should be noted that while Billy starts out eagerly abusing his power and displays every single one of the seven deadly sins while transformed, and he's arguably got far more reason to be bitter at the world, but Sivana is a whole 'nother kettle of worms. Really, Sivana is basically Patrick Bateman.

And you're right. He was already (apparently) a massively wealthy and well-off individual. But it wasn't enough for him. The film does a great job showing that Batson isn't a Saint and has ways of coping with his situation that may not be healthy.
