that and also you discover that orianna runs an orphanage just so that she can feed on fresh blood whenever she wants starting all over from the beginning with a death march run now, wherein i will choose unheroic dialogue options and play much more gwent

After you deal with Orianna, Regis asks very pointedly if you're really sure you still want to do the stupid thing and bother an ancient vampire. At that point you can abort and free Syanna from the fairy tale place. So the 'optimal' route is choosing to go after the ancient vampire, then changing your mind.
