the most infamous one was the 2010 Giro stage to Aquila where a total breakdown in peloton leadership (Astana had the jersey but were down multiple riders due to crashes and didn't want to set pace, and nobody else wanted to either) caused a big group including multiple GC contenders who had lost time on earlier stages to get 15 minutes. This all happened on a miserable, cold/rainy 250KM day with rolling terrain.

Stage 15 of the 2016 Vuelta is probably one of the most infamous of recent times. Results wise it decided the Vuelta on a stage it probably shouldn't have due to some wonderful attacking riding from Contador, but also because it was such an exciting stage with such a drastic effect on GC every other grand tour made sure to throw a stage like it in every year to try and replicate its magic, which has failed drastically every time since.
