Yesterday we had a patient come into the office who’d had his truck roll over his leg while he was working on it. Second time in two months that’s happened. We’ve got him in today for wound care but we have to wear advanced surgical gear because he hasn’t got his covid test back yet.

But we loving saw him in the office yesterday and the hospitals let him in without masks I ahahahahahaha just gently caress it who cares

I don’t have the time or ability to crunch this but I think we should really take North Carolina numbers and project them on the rest of the south east and Texas.

NC did the best out of all of them at locking down (as bad as that is) and there haven’t been any glaring statistic manipulation. I havent heard any stories of that from docs in Asheville or Charlotte.

oh so one of my preceptors like loving cowrote a paper on proning patients with ards and explained to me today why it works

essentially if you look at a CXR of someone with ARDS it's a total white out, but if you look at a CT of the same patient it's the posterior and inferior aspects of the lungs that are primarily effected. This is because we spend eight hours a day on our backs and 16 hours on our feet, so gravity causes all the blood to flow there and consolidations form.

When you put someone to the prone position, you're forcing the blood to flow to the anterior aspect of the lungs that aren't going to have the consolidations, and increase gas exchange.

This is completely new from what I had understood was the MOA for proning patients and thought you guys might find it interesting.

im staying in a converted doctor lounge during the week at this rural hospital where im working. last night i was talking to the guy trying to get the place running again and it turned out we had a possible covid patient. he wanted me to round on her.

full droplet precautions, inconclusive CXR, didn't want to get a CT Scan and risk infecting the hospital.

She had a history of sternum cancer (no idea), costochondritis, various bowel issues (almost ubiquitous out here) and a 35 year ppd smoking history.

Her lungs sounded fine and she didn't appear to be in any acute distress. she had a pretty high fever but I was going in right after she'd been given her q6 tylenol. she didn't have any of the other associated symptoms.

said she was living with two people that HAD it but when she talked to me she just said they KNEW they had it, not tested. That's also pretty common. Everyone out here thinks they had it in like november. Also she'd been to the other hospital in the area 3 times and they didn't do anythin gwith her any of the times.

told her i didn't think she had it but we'd wait on the test. told my preceptor it wasn't a very impressive presentation. went to bed.

oh god she's got covid as poo poo. finally got her CT and it's textbook. AND she's been working for the past 7-10 days at a plant with 700 employees.

threw her on a z pack because we're doing a trial with vandy.

So we poo poo on these assholes a lot but where I am, people literally cannot wear a mask that covers their nose and mouth. They are so sick, from CHF or COPD, they can't function with cloth over their nose and mouth. It is loving insane.

There's a subway near me where I am. It's basically the only restaurant in town that reasonably healthy. Like right now in clinic part of my job is teaching patients how to secure their masks and nasal canulas if they're on o2. And some of these home made masks they have are such poo poo they're closing off their nares so we give them masks if we have some or we instruct them on how to adjust their o2 levels on the fly.

gonna have some rebound hypoxia but what the gently caress else can we do
