My recollection of Japanese healthcare from a course on Post-WWII through Present Japanese Society I took is that while the most basic level of care is cheap, anything above that will cost extra, often paid under the table. Want any doctor other than the most sleep-deprived resident? That'll cost, especially if you want the best doctors at the hospital. Want a private room rather than a bed in an open ward? That'll cost. Better sheets? etc. Also access to specialists is very difficult to come by (having influence and/or connections to the owners of the hospital helps a lot), and for rarer, more complex issues you may well have to look abroad.
