So it is in the best interest of the people high up in a hierarchy (rarely are they actually leaders) for problems undermining that hierarchy to never be identified by anyone at their level or higher. The people at the bottom are the only ones who know how anything works, they are the only ones who really know what does not work and why. The managers just change the inputs and see what happens to the outputs, the actual operation of the organization is a black box to anyone more than one level above the bottom, and is obscured even to the front line managers.
