My guess is that when someone boxes, their model is warped to somewhere on the map before being deleted. Someone maybe didn't realize that that point is actually in a playable area of the map. Something similar happened on Olympus sometimes during dropship, where everyone's player model is actually loaded in somewhere in carrier but you don't gain control until you leave the ship. There was a bug where everyone just spawned in on top of each other and 60 people punched to the death in panic
