I remember that Zack Parsons reviewed a Noboru Iguchi film for the Horrors of Pornography series, and I'd put Shintaro Kago in the same vein. Both of them specialize in erotic horror-comedies, and a lot of their works are in incredibly poor taste. Kago's done some alternative history manga like Dance! Kremlin Palace and Choudouryoku Mouko Daishuurai (The Ultra Power Mongol Invasion), which are funny as poo poo, but messed up.

Shintaro Kago is a gross dude who made a guro comic about a gruesome real life murder.

I think it's creepier we're all about to have opinions about Japanese horror erotica than anything that's been posted so far.

Why would people combine horror and sex anyways? Besides as a short hand for "these deviants are about to die" of course. Wait, why is that a trope too?

There's people that are terrified not only of sex in general, but their own sexuality. Horror and sex have a lot in common aside from increasing your heart rate and sometimes involving ropes.

Horror is very frequently used as a way of subtly addressing more common, everyday fears and insecurities, and a lot of people have hangups about sex. Some people also appreciate using sex and horror together for shock value, as juxtaposition for 'what the audience might like' versus 'what the audience is going to see'. There's also something to be said for the fact that both horror and pornography appeal to base, often illogical reactions in the brain. The two are intertwined on several levels. Alternative joke answer: Fear boners.

Horror, as a genre of film, is descended from morality plays. "Sexhavers get their comeuppance" has been part of horror since before horror was horror.

Yes I know what's in his actual books because I wanted some other ghoulish whimsy like junji ito and was recommended kago without a content warning so reading a few that were merely gross before getting to other ones and seeing their summaries before reading and suddenly realizing "hey... these rest of his works all about forcing people to eat poop "
