As someone who once did a bit of time in the field, a lot of newspapers and publications will have usage rules on courtesy titles like that. For example I believe the New York Times will always refer to a person as "Mr. Smith said..." after the first usage of their name in a story, instead of just as "Smith said..." or "Bob said..." So they most likely edited his letter to conform with existing style rules. The fact that it made a mad pissbaby even madder is just a side benefit.
