There's just this cultural idea in Japan that young people, mostly in middle school and high school, start relationships through a specific declaration of feelings, either in a letter or in person. "Confess" just became the normalized translation because "profess" isn't as popular a word anymore. A vast majority of Japanese to English translation is of anime, where love confessions are a really popular plot device.

Of course, translation is always weird with manga, let along getting into the whole honorific and politeness issues. Given the language apparently has a billion layers of politeness and nuance built around past and present social structures there's an entire genre of comedy about say, referring to a drunken beggar as if he was the Emperor, and/or saying incredibly rude things in incredibly polite ways, etc.

I think anime in particular (though everything else isn't much better) may also have cultivated 'If you don't find your ONE TRUE LOVE in high school or at least while you are young and photogenically attractive you are rejected from society and will be alone and miserable forever' in particular. Though that might also be because it's a particular issue with Japan's society. (which is constructed more or less entirely out of social problems) Also, sorry this got rambly, ranty and wildly off topic. It is supposed to be an exploration of those two extremes. The guy is addicted to porn, his girlfriend is super into sugary romcoms, and they both desperately try to live their respective fantasies through their broken relationship. The film does have problems, but the idea, I thought, was cool.
