Relatively frequently. Firearms are inherently dangerous, and people in the military get way too comfortable around them, which leads to an increase in accidents. Granted, stateside you are very rarely walking around with a rifle and ammo, especially live ammo, which helps mitigate the risks.

I had to deal with an E7 who ND a blank round on a charter flight, more than a week after ammo turn in meaning he hadn't checked or cleared his rifle in at least that long, which started a FAA investigation.

We had a guy ND the 50 cal over the entire fob in Afghanistan. Luckily it was at rest in the turret so it was angled upwards, but who the gently caress knows where that round landed.

We had an ANA guy ND his saw into another ANA guy�s rear end. Round tore up his intestines and killed him.

And I probably saw an M4 ND every few trips to the range.

poo poo happens all the time.

At Benning there was an RTB training range that got used a lot that was right by the road. One ND down the road and there was apparently some words, another ND into my old commanders Humvee driving down the road and that range didn't open back up no more.

There was also a dude at our S1 who nearly got med boarded for TBI after someone NDed a 40mm training round into his head.

There's a LOT of NDs around people who are supposed to do the majority of the shooting.

Whenever I had to carry live rounds or blanks I just never bothered chambering them. No rounds no problems.

A green beret told me about how he almost beat one of his team members to death after he popped a few SAW rounds inches away from his head while disassembling the weapon. The guy who shot the rounds got a trip to Germany to fix his poo poo, and then stateside to have his long tab revoked.

Yeah I purposely left them off that list because, despite their fancy beret and their ability to work with SOF/aircrew, they're solely a support function. And like you said, that runs counter to the belief that being SOF-adjacent means you're SOF (see also: literally everything at Hurby having the naming modifier of "Special Operations", like the Special Operations Security Forces Squadron).
