The level of selfish disregard for human lives necessary to align with US politics on either 'side' is much rarer in other countries and not even getting a lot of money will push your average Scandinavian that way. Russians and Albertans? For sure though. Anyway the US has private hospitals that have plenty of room.

There have been various attempts at getting something like it going but it's slow and the learning curve is steep for new players. It's also not really optimized for anything other than single player. There was an SAS EHM career thing a few years back where everyone got to make players and someone simmed their careers to see how they turned out. That was sorta fun.
