Hard inquires stay on your credit report for two years and lower your score by several points. The effect on your score decays over time and is normally gone in a year so the overall effect is pretty negligible unless you plan on buying a house/car in the next year.

If you can avoid the DOT sending out the ticket by paying it then you'd avoid the administrative fee. Unsure if this is possible in this specific case, but I've avoided the admin fee for a parking ticket by paying the ticket online.

You could easily cut 60k from the number required if you flew business class instead of first, is first class worth ~$1000 and another year of accumulation? You could also apply for the business version of the United Mileage Explorer or the Amex SPG for the signup offers.

Most people wouldn't call that a bonus category. It's 5x for flights booked directly with any airline or via Amex travel. It's not limited to the airline that you choose for the fee credit. You should be able to book award tickets on Hawaiian via Korean by calling. It’s a different chart than Skyteam though, iirc 30k miles r/t for economy.

MCT will give you plenty of time to connect if the flight isn't delayed. I'd guess the MCT is 30 - 60 so you're probably double it giving you some buffet for delays too.

If you ignore the lake area then the only two things in Milan that are worth a stop are the Duomo and the Last Supper. There's a nice canal area near Conchetta that's you can hang out in for aperitvo and other random sites, but most other Italian cities have more to see in my opinion.
