I remember when Undertaker was having his original squash matches on Superstars, Honky Tonk Man on commentary dubbed it a Tombstone since he'd just killed his opponent. Although who knows where he stole it from.

For the WWE championship (and discounting Daniel Bryan who won his world title off a cash in), Sheamus at TLC 2009. He won his title opportunity in a battle royal. For World Championship, Mark Henry at Night of Champions 2011. Think every following champ has been a repeat or a MitB cash in.

God, just the proportions involved in those two should have made it immediately obvious. There was no room for Nash to properly get his arms around Giant, near his waist, and no way for Giant to even try to lift himself up from that angle.
