FISH CHAT

So I got my son a 20 gallon fish tank and we have been putting native plants and fish in it and it's p cool.

Watching fish swim about is super chill. My dad used to have a tropical tank and my favourite fish were the tiny catfish, they're rummage around the substrate at the bottom like little dogs. We had some upside down ones too, they'd swim about upside down, sleep upside down under leaves and only flip right side up to feed. Photos of the tank, yo.

BajaHAHAHA.

Clown loaches were my favorites when I kept fresh water tanks. They were fun to watch and the analogy to tiny dogs is apt as they definitely had personalities.

I dabbled in salt water nano setups and let me tell you, that way lies madness. Incredibly interesting, colors popped like nothing I've ever seen with fresh water, and the sheer variety and different scales upon which life flourished in the tank was mesmerising. Incredibly dense in terms of $$$ per gallon, upkeep was a lot of work, and tolerances for conditions were tight, but while I had it I loved it.

Turns out there are a lot of wild tropical fish in FL, we found a pond loaded with Mollys, put some in the tank and they immediately started having babies. Also got a baby Tilapia and a bunch of shrimp. They are doing way better than I thought they would :)

The Tilapia is actually pretty cool, he's got personality :v:

We're building a fish trap to catch some really pretty adult male Mollys because they are too hard to catch with a net. It's easy to catch the pregnant females tho :3:

The V8 chrome rendering engine only loads so far up and below what you're viewing, so if you have to scroll to my post it will render the gifs starting at different times based on how fast you're scrolling.

I debated changing it (and spent the $5) to this, but I changed it back.

Genital chat: I signed up for a patient two nights ago whose chief complaint was rash. The triage note said, "Patient states, 'i got something going on down there, I don't know if bedbugs are eating my nuts but it's all hosed up.'"

I walk in the room and as the guy what's up and he starts talking about his ankle. I'm only half paying attention -- this happens all the time, where people talk about something innocuous and then add, "But let me be honest, doc, what I'm really here about is (enter genital horrorshow here)." loving all the time. If someone shows up at 0300 with a blank chief complaint, it is always an STD check.

But he doesn't bring it up. And then the x-ray tech walks in. I check his bracelet and they put the wrong guy in the room, while still putting in the plain films for his ankle sprain under the other patient's name. I told the patient about the error and then said, "I was starting to get confused 'cause this was nothing like the nursing note, and I thought this was just a really long lead up to you telling me about your nutsack!" We all laughed. Good times were had. I looked at no taints that night.

I swear, 90% of what we do is tedium and 5% is sheer terror, but the remaining 5% is loving hysterical. (The 5% of sheer terror includes when an attractive young woman tells you she's having pelvic pain, denies vaginal bleeding or discharge, and then you do the pelvic examination and pus is literally just pumping out of her cervix like she's been storing it up there for a few days. How did you not notice this?!)

When the sample is composed of "women presenting to the emergency department for isolated pelvic discomfort," the pre-test probability of this physical exam finding is exceptionally high. I call this "Toxic Box Syndrome."

(Most pelvic exams are not this bad. But the combination of an otherwise attractive, clean-appearing young women plus surprise necrotizing coochitis just makes me so glad I was careful in college.)

I got put in the wrong room with my GP-equivalent for a routine checkup a few months ago. Nurse comes and tells me to take off my clothes for a physical so I'm sitting in my boxers when the doc comes in.

Good stories are sadly more rare than the banal (thirty visits for reflux in two years), annoying (drug seekers lying), or devastating (you have cancer!). My absolute favorite patient encounter, which I will post when I'm not on my phone, was "I fell off a dirt bike and I need an STD check."
