in theory it's giving a major assist to your immune system in clearing it out. but covid could have already hosed your immune system pretty good by day 5 of symptoms, and if it's still around after the course, it can go back to attacking it some more.

in my non-scientist opinion 5 days is absolutely a mistake and significantly too short.

Sorry, just have to rant, I'm furious. I spent the entire morning yesterday responding to my dad with a massive, detailed email full of stats and numbers and links to sources and explanations. I'm full of self-loathing but it was a good, informative email that stuck to facts and took emotion out of it. Just heard back.

Every number, every point I made, the entire thing - completely ignored. Maybe I should have pasted it into Occupy Democrat format?

90% of the email dealt with asymptomatic and "mild" cases causing Long COVID, and he's still separating them out from Long COVID. And putting Long COVID in quotes was a nice touch too. I'm livid right now.

I'm terribly sorry. I've been there.

Bottom line is, you cannot save this guy from himself. Because, at bottom, nobody can save anyone from themselves.

it took me a good two decades to actually learn this loving lesson. I didn't want to. It's not easy to accept, especially when you care for the person who's trying to self-destruct.

And it really sucks to learn it this personally.

But another bottom line, a moral one, is that you gave it your best shot. You didn't just walk away, without even trying. So please please don't blame yourself if he goes south. I've seen that play out too, and it doesn't go anywhere good.

yeah, the problem is that in brasstacks paxlovid is not directly reducing the amount of virus in your body (as some mAbs do like, binding virions directly), it's stopping there from becoming more virus in your body, which leads to a knock on "assist" to your immune system conducting its typical function and hopefully not being overwhelmed.

looking back the "must administer ASAP" thing makes this clear, because the overall net viral load can be stopped at the slower part of the exponential curve lol

the fact that so many studies consistently find that female sex is a huge risk factor for long covid makes me almost certain that some of the large/ongoing symptom clusters are the result of autoimmune attacks, which tracks with the 2:1 sex disparity in autoimmune disorders generally. ouch

in theory, the total amount of virus should still decline, because the X number of virions in your body will be constantly entering new cells, where they shed their protein coating (never to be used again), inject their RNA, and then the RNA dies after it produces proteins which are then prohibited by paxlovid from being cut into the necessary replicases to make new virions, meaning some percent of those X die without reproducing. apparently enough to reduce it safely below X from the efficacy numbers

but still it is a huge problem that like, if your immune system is bad and any virions survive the 5 day course, there's nothing stopping them from coming back.

i assume that for people with bad immune systems, they may need to be on paxlovid for a long time, or even just lifelong if it's bad enough.

Thanks, I appreciate it, and I'm sorry you've had to go through it too. It could be a lot worse, but I just keep thinking about their being maskless indoors all day every day with my sister's family. They're all going to get it multiple times, and given all of my mom's weird health issues, it's hard not to anticipate the worst happening sometime soon. Can't help but think of all those stories of people unexpectedly losing family members to MAGA in 2016.
